Our integrated services
Exceptional Minds Healthcare LLC delivers integrated Home and Community-Based Services (HCBS) in alignment with the Arizona Division of Developmental Disabilities (DDD) and AHCCCS requirements. Our service model is person-centered, strength-based, and designed to support members in the least restrictive environment while promoting independence, health, and community inclusion.
Services are coordinated through the Individual Support Plan (ISP) and implemented in accordance with Arizona Administrative Code and DDD policy guidelines. We provide habilitation, attendant care, respite, and behavioral health support through structured teaching strategies, measurable objectives, and ongoing data collection to ensure progress and accountability. Our interdisciplinary approach promotes continuity of care, safeguards member rights under Article 9, and ensures compliance with HCBS settings requirements while addressing intellectual, developmental, behavioral, and physical health needs in a unified service delivery model.
Habilitation
Habilitation is a skill-building service designed to help individuals develop, maintain, or improve daily living and social skills. Services are individualized and may focus on communication, self-care, behavior regulation, community integration, safety awareness, and independent living skills. Habilitation promotes growth, confidence, and long-term independence through structured teaching strategies that are person-centered and goal-oriented
Attendant Care
Attendant care supports individuals who need assistance with daily living activities due to physical or developmental limitations. This service includes help with personal care tasks such as bathing, dressing, grooming, toileting, meal preparation, mobility, and basic household tasks. The goal of attendant care is to promote health, dignity, independence, and participation in daily routines while respecting each individual’s preferences and abilities.
Respite
Respite care provides temporary relief for parents, guardians, and primary caregivers who support individuals with developmental disabilities. This service allows caregivers time to rest, attend appointments, work, or manage personal responsibilities while ensuring their loved one remains safe and supported. Respite can be provided in the home or community and focuses on supervision, safety, and continuity of care in a familiar and respectful environmen
Home Community Based Services
Home and Community-Based Services (HCBS) are supports provided to individuals with developmental disabilities in their own home or community rather than in an institutional setting. In Arizona, HCBS are part of the Arizona Division of Developmental Disabilities (DDD) service system and are funded through Medicaid/AHCCCS. These services are designed to promote independence, dignity, and full community inclusion by helping individuals live safely and meaningfully in their chosen environment. HCBS may include habilitation, respite, attendant care, and other individualized supports based on a person-centered plan. The goal is to ensure each member has choice, control, and active participation in everyday life.
